Lost ID
This service provides information on how to process or request replacement of student ID.
Processing Time
On regular days, it takes about 30 minutes to process a new ID replacement.
During peak seasons, like enrollment, processing may take up to two (2) - three (3) working days.
Service Cost & Payment Options
PhP 300.00 / replacement
The replacement cost is only for the ID card itself. Lanyards and casings are available at the Bookstore.
Eligibility
Only enrolled students for the current term or active employees with the appropriate affidavit of loss are eligible for this service.
When can you avail of the service?
The service is available from Mondays till Saturday 8:00 AM till 5:00 PM. Noon breaks apply.
How to apply for the service?
Eligible persons who lost their ID must follow these steps.
For Students
- Proceed to the Office of the Prefect of Discipline (OPD) for Intramuros or to the Center Student Activities and Discipline (CSAD) for Makati
- Student must ensure to have the affidavit of lost notarized and duly acknowledge by OPD or CSAD
For Employees
- Proceed to the Human Resources Department (HR) and file the affidavit of loss.
- HR shall acknowledge the affidavit of lost
After the appropriate steps above:
- Proceed to the ID Picture taking
- For Intramuros : The ID Picture taking is at the 4th Floor Main Building
- For Makati : The ID Picture taking is at 2nd Floor at the Open Laboratory
- The personnel in charge will advise when the ID will be available for pick-up
The student shall be charged the appropriate ID Replacement Fee.
Delivery Method
ID’s must be picked-up where they applied.
- For Intramuros : The ID Picture taking is at the 4th Floor Main Building
- For Makati : The ID Picture taking is at 2nd Floor at the Open Laboratory
