Student lost his/her Report Card and previous school does not provide a copy anymore

Student must provide the following documents:

-Affidavit of loss
-Certified True Copy of Form 138 (School report card)
-Certification from the high school principal regarding the student’s Form 137-A. Contents of the certification are as follows:
The student’s Form 137-A is still in the school file
The student’s F137-A has not been forwarded to another college/university

And please take note that all hard copies of the documents must be submitted on the submission date provided for your enrolled program.
