Correction of Personal Information
This service allows the student to request corrections for personal information such as name, date of birth, and the likes which are non-editable in the portal since these are controlled fields and are related to external documents.

This process is typically.processed within two (2) business days upon receipt of the request.

Eligibility
Any student can avail of this service provided that they have the necessary documents that support it. Some examples of these documents are as follows;
- Birth Certificate with the Philippine Statistics Authority (PSA) security paper
- Alien Certificate of Registration (ACR)
- Affidavit of legal age
- Affidavit of two disinterested parties
Note that changes in the students’ name will not be applicable if the student has already graduated or transferred out of the university.
When can you avail of the service?
Students may file their request at the customer service section of the Registrar’s Office Monday-Saturday 8am-5pm as this requires physical validation of the submitted documents.
How to apply for the service?
Any student who wish to update their profile must follow the steps below;
- Download and accomplished the correction of name form and submit the supporting documents to customer service section of the Registrar’s Office.
- The Registrar’s Office shall review the submitted documents and if everything is in order, the system will be updated.
