Request for Transcript of Records

A transcript of records is an official academic document issued by the university that lists a student’s courses, grades, credits earned, and sometimes honors or distinctions. It serves as a formal record of a student’s academic performance and is often required when applying for jobs, scholarships, or further studies.

The university only issues transcript of records directly to the following;

  1. Student or Alumni
  2. Authorized Representative - with the necessary authorization documents
  3. The requesting school in cases when the student transfers or takes up another degree from a different school. Please refer to the knowledge base articleTransfer Credentials for more details on transferring schools.

Processing Time

Transcript of Records typically takes 15-20 working days or less to process. This depends on a number of factors affecting your academic records like its archival state, record availability for verification, and other factors.

Service Cost & Payment Options

The following processing fees are charged to your account.

  1. Processing Fee - PhP 300.00 per set
  2. Mail To / Courier Fee - these are additional charges intended for courier service
    1. Within the Philippines - PhP 250.00
    2. International Address - PhP 4,160.00

Eligibility

Any undergraduate student, graduate program students, or alumni can request for their transcript of records. If the request is to be made by a representative, an authorization letter with a scanned copy of any government ID of the student or alumni is required.

Senior High students transcript of records is know as Form 137A (F137A) and follows a different process. Kindly refer to knowledge base article Request for F137A for more details.

When can you avail of the service?

Request is available anytime.

How to apply for the service?

The requestor will need to follow these steps;

  1. File the appropriate request form (see below).
    1. Undergraduate Programs
    2. Masters, Doctorate, Post Doctorate Programs
  2. Endorse the filled-out form to the customer service section or the Registrar’s Office or email it to the following;
    1. For Intramuros Programs: maangulo@mapua.edu.ph
    2. For Makati Programs: jlayco@mapua.edu.ph
  3. Your account will be charged with the processing fee as well as any courier services if availed.
  4. You will be notified via email if the document is ready for pick-up or has been sent out via courier

Delivery Method

In your application form, please don’t forget to specify the delivery option for the duplicate copy of the diploma.

  • Mail to - the document shall be sent via courier in a sealed envelop. Additional charges may incur for the service
  • Pick-up - please pick-up the document at the customer service
